The Care Management Program purchases in-home services on a pre-set, per-unit basis according to an individual’s care plan and a Direct Service Purchase (DSP) Provider Enrollment Agreement.
Providers bill monthly to be reimbursed for services provided. Services provided include Community Living Supports (In-Home and Residential), Personal Care, Homemaker, In-Home Respite, Out-of-Home Respite, Chore, Home Delivered Meals, Personal Emergency Response Systems, Counseling, Financial Management, Nursing Services, Private Duty Nursing,Medication Management, and Non-Emergency Medical Transportation.
All Residential settings (eg. AFC, ALF, HFA) must be in compliance with the Michigan Department of Health and Human Services (MDHHS) Home and Cummunity Based Settings (HCBS) assessment prior to being accepted as a provider of services for Region IV Area Agency on Aging.
