Coordinating care in the home
The MI Choice (pronounced My Choice) Waiver program is designed specifically for low-income adults age 18 and older who need nursing home level of care but prefer not to live in a nursing home. The program provides individuals with a choice to receive the services they need in their own home or other community setting such as an assisted living or adult foster care home instead of in a nursing home. A Realistic Long Term Care Choice:
Region IV Area Agency on Aging offers adults of any age the option of receiving long term care in the home and community instead of a nursing home or other long term care facility.
The Care Management program offers an in-home assessment and connection to an array of home and community-based services to assist older adults and younger persons with disabilities to live in the setting of their choice. It can also help people transition back into the community from an assisted living facility, nursing home, or after a hospital stay.

In-Home Community Services assists seniors and adults with disabilities to live in their own homes and communities with dignity and independence. Various funding sources have differing eligibility requirements. Some programs are based on age and income, others have are-only eligibility requirements.
