My Life. My Choice
Coordinating care in the home

The MI Choice (pronounced My Choice) Waiver program is designed specifically for low-income adults age 18 and older who need nursing home level of care but prefer not to live in a nursing home. The program provides individuals with a choice to receive the services they need in their own home or other community setting such as an assisted living or adult foster care home instead of in a nursing home. Income and asset limits apply. Please call (800) 654-2810 to learn more.

My Choice Waiver

For qualified individuals, the MI Choice program will help cover services such as personal care, homemaking, adult day services, in-home respite programs for the caregiver, meals on wheels and personal emergency response systems. To receive a copy of our Provider Directory, click here
