We are seeking a skilled and empathetic Advanced Practice Provider (APP) who shares our passion for delivering high-quality, person-centered care — right where health happens: in the home.

As part of our Integrated Care at Home team, you will work in partnership with patients, families, and a multidisciplinary team to improve health outcomes, safety, and quality of life. Whether in private homes or adult foster care settings, your role will be rooted in trust, transparency, and a deep commitment to meeting people where they are — physically, emotionally, and medically.

POSITION SUMMARY: Utilizing a holistic model of care founded in the integration of medical and social care, delivers compassionate, person-centered medical care and education to improve health outcomes, engagement, safety, and quality of life in home and community-based settings such as adult foster care homes. Collaborates with a multidisciplinary team inclusive of nurses, social workers, behavioral health counselors, community health workers, dieticians, and the medical director to help patients remain healthy and successful in their homes. Function within their applicable Scope of Practice, medical standards of care, and collaborative agreement with physicians, including diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Creates positive rapport and engagement with patients and coworkers while maintaining a focus on excellence in customer service.
  • Performs in-depth clinical assessments on all assigned patients by completing encounters inclusive of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Excels in clinical documentation and ensures all charting is completed accurately and promptly.
  • Reviews and interprets the medical, surgical, and social history of patients and performs appropriate screenings.
  • Synthesizes information, establishes diagnoses, and leads the development of the Plan of Care with patients and their support system, and regularly reviews and updates the Plan.
  • Educates patients and families on disease processes, medications, symptom management, and health maintenance while advocating for services and resources that support patient goals and safety in the home.
  • Participates in patient care meetings as requested and collaborates with multidisciplinary team members.
  • Interprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up.
  • Consistently reconciles medications and prescribes or performs medically indicated treatment modalities.
  • Refers patients to specialists and/or ancillary services as indicated.
  • Works collaboratively with the medical director, physicians, care coordinators, staff, family members, key caregivers, and any appropriate ancillary medical personnel.
  • Identifies overall quality improvement activities through assessing and planning health care individualized to the patient and consistent with policies and protocols.
  • Maintains compliance with OSHA and state licensing boards and participates in all mandatory compliance and other training programs.
  • Practices in accordance with applicable local, state, and federal laws which govern confidentiality and medical information privacy regulations (HIPAA).
  • Provides constructive and timely feedback about recommendations to improve and/or streamline operations.
  • Participates in continuous quality improvement activities and special projects as requested.
  • Communicates and collaborates with leadership to achieve patient care and organizational needs and goals.
  • Consistently meets key performance indicators as established with the program manager.
  • Maintains strong relationships with partners and providers throughout the health and human service community.
  • Other duties as assigned.

SUPERVISORY RESPONSIBILITIES: This position does not have direct supervisory responsibilities.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required. Local travel required; must have reliable transportation and provide current proof of automobile insurance.

EDUCATION AND/OR EXPERIENCE: Graduate degree in Nursing (Nurse Practitioner) or Physician Assistant Studies required. Must possess baseline clinical knowledge; experience in clinical practice preferred.

REASONING ABILITY: Ability to accurately assess patient conditions, collect and interpret clinical data, establish facts, and draw valid medical conclusions. Ability to synthesize complex medical information and develop appropriate treatment plans. Must demonstrate sound clinical judgment and ability to work with abstract medical concepts and multiple variables.

LANGUAGE SKILLS: Excellent oral and written communication skills required with the capability to listen effectively and work well with patients, families, and healthcare team members. Ability to read, analyze, and interpret medical journals, clinical reports, and legal documents. Ability to respond to inquiries from patients, families, regulatory agencies, and healthcare providers. Ability to effectively present clinical information to the medical team, management, and patient families.

MATHEMATICAL SKILLS: Ability to work with mathematical concepts, including medication calculations, statistical data interpretation, and clinical measurements. Ability to apply concepts such as fractions, percentages, ratios, and proportions in clinical situations.

OTHER SKILLS AND ABILITIES: Must have computer skills, including use of email and Microsoft Office. Must effectively utilize time and navigate change and uncertainty. Must establish and maintain effective relationships with patients and their support systems. Good clinical judgment must be used, as well as the ability to deal with confidential medical information. Must effectively manage clinical caseload and documentation requirements.

LICENSURE/CERTIFICATIONS:

  • Must possess and maintain a valid Michigan driver’s license.
  • Must possess and maintain current state licensure as either a Nurse Practitioner or Physician Assistant.
  • Must hold current national certification appropriate to discipline (AANP/ANCC for NPs; NCCPA for PAs) in relevant specialty area (Adult, Family Practice, Geriatrics or Adult-Gerontology Primary Care).
  • Must have and maintain Basic Life Support (BLS) certification.
  • Must test for tuberculosis upon hire and review symptoms annually.

PHYSICAL DEMANDS:

While performing the duties of this job, the employee is regularly required to communicate and express oneself clearly with patients and healthcare team members. The employee frequently operates a computer and medical equipment and positions themselves to perform clinical assessments and procedures. The employee is frequently required to remain in various positions during patient examinations and to move between patient homes and clinical locations. The employee must be able to travel in various weather conditions to patient homes.

While performing the duties of this job, the employee may need to move up to 15 pounds regularly and occasionally move up to 30 pounds of medical equipment and supplies. Specific vision abilities for this job include close vision for clinical assessments, depth perception, distance vision, and the ability to adjust focus for various clinical tasks and documentation.
