We’re inviting an experienced and mission-driven Advanced Practice Provider (NP or PA) to join our Integrated Care at Home team. This is an opportunity to practice medicine the way it was meant to be practiced—through relationships, thoughtful care planning, and a deep understanding of each person’s whole life, not just their diagnoses.

What You’ll Do

  • Conduct in-home clinical assessments and medication reconciliation
  • Develop and regularly update Plans of Care in partnership with patients and caregivers
  • Diagnose, treat, and manage acute and chronic conditions within scope of practice
  • Educate patients and caregivers on conditions, medications, and symptom management
  • Collaborate with the Medical Director, interdisciplinary team, and community partners
  • Maintain strong clinical documentation and adherence to compliance and continuous quality

Qualifications

Required:

  • Current licensure as a Nurse Practitioner or Physician Assistant in the State of Michigan
  • Practice independently with strong assessment, diagnostic, and care-planning skills
  • Excellent communication & relationship-building abilities with patients, families, and teams
  • Valid driver’s license with ability to travel throughout Berrien, Cass, and Van Buren counties

Preferred:

  • Experience providing care in home-based, community, or adult foster care settings
  • Background in primary care, geriatrics, chronic disease management, or integrated care
