GUIDE Model

The GUIDE (Guiding an Improved Dementia Experience) Model is a new payment approach for key supportive services furnished to people living with dementia, including:

  • Comprehensive, person-centered assessments and care plans
  • Care coordination 24/7 access to an interdisciplinary care team member
  • Certain respite services to support caregivers
  • Caregiver assessment
  • Care Navigator to help access clinical and non-clinical services such as meals and transportation through community-based organizations.

Eligibility:

  • Has mild, moderate, or severe dementia
  • Enrolled in both Medicare Parts A and B
  • Has Medicare as their primary payer
  • Is not enrolled in Medicare Advantage or another Medicare health plan, including Special Needs Plans (SNP) and PACE
  • Is not enrolled in Medicare hospice benefit Is not a Long-Term Nursing Home resident

Caregiver Support:

The GUIDE program provides a caregiver assessment to evaluate their knowledge, needs, social support, well-being, stress levels, and other challenges. The GUIDE Model provides respite services, enabling caregivers to take temporary breaks from their caregiving responsibilities. Respite is being tested under the GUIDE Model to assess its effect on helping caregivers continue to care for their loved ones at home, preventing or delaying the need for facility care.
