We are inviting an experienced and mission-driven Information & Access Specialist to join our team. This role is essential in connecting seniors with the services they need, while ensuring smooth access to healthcare resources.

What You’ll Do

POSITION SUMMARY:

  • Point of Contact – Act as a friendly, informed, and supportive resource for individuals, caregivers, and professionals seeking help with aging, disability, caregiving, and health-related social needs.
  • Conduct health-related social needs screening/assessment and other person-centered conversations to identify priorities, barriers, and potential eligibility for programs, services, and support.
  • Provide navigation across community, healthcare, and service systems, supporting individuals through eligibility, enrollment, scheduling, and other access-related processes.
  • Provide follow-up and problem-solving support to help ensure recommended services and resources are successfully accessed, addressing barriers that may arise along the way for high-risk individuals.

Qualifications:

  • Clear and compassionate phone skills
  • Capable of identifying gaps in services and addressing client needs
  • Knowledge of healthcare and community support systems
