Location: Campus for Creative Aging, 2920 Lakeview Ave., St. Joseph
Walk-In Wednesdays are limited to individuals or couples. Family groups of two or more are encouraged to make an appointment due to time needs.
You are required to bring your Medicare card and list of medications (full medication name) or actual medication to appointments and walk-ins. Not having these items will result in not being able to put your name in the queue or loss of appointment with limited time to reschedule.
Volunteer MMAP Counselors are available.
