The Care Management program offers an in-home assessment and connection to an array of home and community-based services to assist older adults and younger persons with disabilities to live in the setting of their choice. A helping hand is here.
MI Choice
It’s your life. Where you receive supports and services should be your choice. The MI Choice Waiver program is designed specifically for low-income adults age 18 and older who need nursing home level of care but prefer not to live in a nursing home.
Leaving the Nursing Home?
The Nursing Home Transition Program is for people who reside in a nursing home and want to talk with someone about other living options. If you are interested in moving out of the nursing home, we are here to help.
