MI Choice Waiver

It’s your life. Where you receive supports and services should be your choice.

The MI Choice (pronounced My Choice) Waiver program is designed specifically for low-income adults age 18 and older who need nursing home level of care but prefer not to live in a nursing home. The program provides individuals with a choice to receive the services they need in their own home or other community setting such as an assisted living or adult foster care home instead of in a nursing home. Income and asset limits apply. Please call (800) 654-2810 to learn more. Trained specialists will walk you through your options so you can make an informed choice.

For qualified individuals, the MI Choice program will help cover services such as:

  • Adult day care
  • Caregiver relief
  • Care Management
  • Help with bathing, dressing and transferring
  • Home-delivered meals
  • Light housekeeping
  • Laundry
  • Non-medical transportation
  • Non-emergency medical transportation
  • Nursing services
  • Personal Emergency Response System (fall button)
  • Specialized equipment

Those enrolled in the program may use a home care agency or hire and supervise home care worker(s) of your choice.

To refer yourself or someone else to MI Choice Waiver or other Area Agency on Aging services online, complete the Referral Form.
