Integrated Care at Home services are available to individuals aged 18 and older with a disability or chronic health conditions who would benefit from receiving their care and services in their home.
This home-based medical care program is designed to lessen emergency room visits and hospitalizations, and bring health to where it happens – home. Both Area Agency on Aging and Integrated Care at Home serve Berrien, Cass and Van Buren counties.
Services provided:
- Primary Care: A nurse practioner can provide annual wellness visits and address any health-related needs.
- Behavioral Health Counseling: Talk with a licensed counselor to work through a wide number of mental health condition such as anxiety, depression, phobias, and addictions.
- Chronic Care Management: A Community Health Worker can assist in achieving your heath goals by helping to coordinate home and community-based services.
- Nutrition Counseling: Work with a register dietician who can translate the science of nutrition into practical solutions for healthy living.
The team of providers is supported by a robust team of social workers, nurses and administrative staff at Area Agency on Aging.
How do I qualify?
– 18 years or older
– Primarily homebound or have limited ability to get out of the home to get to health care appointments
– Managing a chronic condition like diabetes, chronic kidney disease or heart disease
How do I pay for services?
Integrated Care at Home accepts most health insurance plans, including Medicare and Medicaid.
