Enrollment Facilitator

Description:  

Work closely in collaboration with other members of the Care Management and Information and Access Departments to aid in applicants’ access to and enrollment in Region IV AAA programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Telephones and/or visits program applicants. Completes additional screening to determine program eligibility. Assists in gathering financial documentation for Medicaid application.
  • Corresponds and coordinates with Information & Access and Care Management staff, clients, their representatives, businesses, community service providers, and others to access information needed to complete entitlement applications.
  • Provides support to applicants working toward eligibility. Connects applicants with additional community services as appropriate.
  • Documents all activity in appropriate software system(s).
  • Assists as needed with other projects and assignments that optimize program delivery.

REQUIREMENTS:

High school diploma or general education degree (GED); or one-year related experience and/or training; or equivalent combination of education and experience. Computer literacy in routine office applications required. Long-Term Care Medicaid knowledge preferred.

TERMS:
